Note on tax: The structural Iranian Direct Taxes Act progressive bracket runs from 15 percent at the first 1.2 billion IRR annual to 30 percent above 8.4 billion IRR annual. The local public system in Tehran runs the central public hospital tier at the structural Tehran Heart Center, the Imam Khomeini Hospital Complex affiliated with Tehran University of Medical Sciences, the Shariati Hospital, the Pars Hospital private network, and the Atieh Hospital private tier. The system is structurally serviceable at the top tier hospital network and crowded at the community clinic tier. Who should move to Tehran, and who shouldn't.

Tehran is the capital that the sanctions and the internet restrictions reshaped, and the index score reflects it.
